Online IntakeOnline Intake is available for those wishing to apply for LSNF services. Each applicant must submit a fully completed application for LSNF to determine eligibility for services. Before beginning the online intake process, be sure to have the following information with you so you can complete the application:
- Mailing address
- Citizenship status
- Social Security Number
- Number of people living in the household
- Monthly income of each adult living in your household including yourself
- Monthly expenses of each adult living in your household including yourself
